Through our Crisis Stabilization Unit (CSU), Staff Secure Residential Unit (SSR), hospital diversion program (New Vistas) and Jefferson Hills Academy, we use some of the most current practices, interventions and protocols to address the complex needs of vulnerable youth and their families.

Case Manager Position Purpose:

The Case Manager is assigned to a residential unit and provides the case management services for the residents of that unit. He/she coordinates services and case conferences with the treating therapist, the representatives of the agency that referred the clients to Jefferson Hills, and residents’ families. Responsibilities include:

  • Maintains data which tracks every youth’s progress and potential discharge

  • Schedules family therapy sessions, case conferences and other related meetings for each resident

  • Coordinates after care appointments to ensure ongoing care and treatment of residents post discharge.

  • Documents all contacts and appointments in the resident’s electronic medical record.

  • Works with multidisciplinary teams to determine length of stay and insurance coverage

  • Coordinates family visits

  • May attends and participates in weekly clinical rounds/staffing as well as weekly team meetings and clinical team meetings

  • Creates and maintains accurate, timely records and reports using an electronic medical record and other databases

  • Maintains good communication and builds solid working relationships with treatment team members, therapists, milieu staff, program managers, parents and external resources to receive and provide professional support and pro-active problem-solving strategies.

Education/Requirements/Skills:

  • Bachelor’s degree in social work, psychology or related discipline.

  • Case management experience in a mental health setting including after care services with external providers

  • Demonstrated ability to work effectively and with empathy with families in crisis situations; professional demeanor.

  • Experience working as part of a multidisciplinary team.

  • Strong organizational skills and detail orientation; able multitask and handle competing priorities

  • Excellent computer skills including working with an electronic medical records system.

  • Experience in mental health or related field in inpatient settings, crisis prevention, juvenile corrections, or related field.

  • Provisional License through the Department of Regulatory Agency may substitute for some experience.
